JOB TITLE: Medical Assistant
LOCATION: Putneymead group Medical Practice
HOURS OF WORK: 37.5 hours per week
SALARY: £12.75 per hour
ACCOUNTABLE TO: Chief Operating Officer
Objective:
Work with a clinical team to support our cohort of patients with long term conditions. This will involve ensuring a smooth patient journey through the practice pathways, by:
- Effective and professional communication with our patient population through a variety of methods
- Collate patient conditions ensuring they can get the support required to manage their condition in the most efficient way
- Support the coordination of the patient journey by working closely with our multi-disciplinary team to determine the best route in the best interest of the patient

- It is essential that the post holder is able to demonstrate an ability to work autonomously as well as part of a team.
- An ability to demonstrate initiative, empathy and problem-solving skills, along with strong communication and negotiation skills at all levels.
- An ability to engage in a professional manner applicable to this role.
- To provide support and integration with other colleagues and teams within the organisation to work collaboratively with different staff groups i.e. Physicians Associates / Clinical Pharmacist / Reception team and Management Leads.
Duties & responsibilities
- Long term condition reviews
- Booking patient appointment in line with their condition with the appropriate clinician
- Communicating with patients effectively via phone, text messages and email
- Coding/Tagging clinical documents in clinical system
- Coding and attaching documents to patient records according to their type.
- Accurate data entry into patient records
- Alerting appropriate clinicians concerning actions stated within document
- Coding/Workflow clinical documents
- reading through documents received and coding readings in patient records
- reading through documents and alert/workflow to appropriate clinicians per GP actions stated in document
- Monitoring and managing EMIS Amendments
- reading through amendment alerts and researching whether suggested amendment is correct
- Contacting patient, if need be, in confirming if suggested amendment is correct
- Supporting with managing patient deduction process
- Supporting colleague in charge of conducting deduction process and mailing files of patient records a s appropriate
- Support with coding Event Degrades in EMIS
- Support with managing Integration failures and Integration Successful alerts within clinical system
- Filing physical files or pulling physical files:
- Supporting with filing physical files
- Supporting with pulling physical files
- Prescription Processing to include EPS
- Any other duties deemed reasonable
